If you’re a current Forward member you can schedule your Baseline visit or all of your follow up visits through the Forward mobile app. If you haven’t already, please download the “Forward” health app in your app store and select “Get Started”.
Then, follow the steps on the app to continue through your on-boarding process. Otherwise, open your Forward app and chat with your Care Team about what’s happening.
