Your Forward membership is active as soon as you sign up and pay for your first month.

Once you've signed up, the next step is to download the Forward app ( and schedule your Baseline visit.

The Baseline visit is your inaugural visit with Forward. You'll spend quality one-on-one time with your Doctor discussing your past medical history, current health and wellness goals, reviewing lab work and performing a thorough physical exam. At the end of your Baseline visit, you will work with your Doctor to build a personalized care plan.

As soon as your Baseline visit is complete, your ability to utilize the Forward app will be fully unlocked. You will be given access to the plans you built with your Doctor through the app. You will also have access to 24/7 chat with your care team.

You will also use the Forward app to schedule any in-person or telephone followup visits that you may want throughout the course of your membership. You're not limited to a certain number of chats or visits! You can contact us through the Forward app anytime—day or night—to help with whatever is on your mind. Whenever you need your care team, we're here for you.

Because the app also gives your doctor and team a way to reach out to you, we're able to be proactive about things like reminders, offers to help schedule outside visits that we might recommend, or visit requests to have you come in again to see your doctor to, perhaps, review your genetic testing results.
